Clinical experiences in the PGY2 Ambulatory Care Pharmacy Residency will include core practice experiences that will be longitudinal in nature and focus on direct patient care in the Patient Centered Medical Home (PCMH) model and elective rotations tailored to the individual’s interest. Elective rotations are also longitudinal, and include opportunities in anticoagulation, cardiology, chronic kidney disease, hematology/oncology supportive care, oral chemotherapy, psychiatry, solid organ transplant, and transitions of care. A multitude of teaching opportunities are available to the resident, who will receive a faculty appointment as a Clinical Instructor at the University of Michigan College of Pharmacy.
