This specialty residency in Ambulatory Care is designed to train pharmacist practitioners to attain mastery in providing direct patient care to patients with chronic disease (diabetes, dyslipidemia, hypertension and heart failure). The clinical sites include two urban, safety-net, free clinics and university subspecialty clinics that employ collaborative drug therapy management by pharmacist providers.The underserved populations include broad exposure to non-English-speaking patients with an emphasis in Spanish at one site. In addition to excellence in a variety of clinical skills, this program will also successfully prepare residents considering a career in academia. A unique aspect of this residency is the opportunity to participate in ongoing evaluation of innovative practice models.
