DETAILED INFORMATION

This residency offers an exceptional learning opportunity within an integrated safety net health system. The program goal is to train ambulatory clinical pharmacists to become proficient in the design and delivery of clinical pharmacy services as part of an interdisciplinary team. The resident will provide comprehensive disease state management and population health services both in clinic and via telephone within family and internal medicine clinics. The program is structured into quarter blocks, and the resident will have 2-3 rotational experiences each block either in a required and/or elective setting. Applications must be submitted via PhORCAS: Letter of intent; Curriculum Vitae; 3 letters of reference (at least one from an ambulatory care preceptor); and college of pharmacy transcript. Program has ASHP candidate status.
