DETAILED INFORMATION

Collaborative primary care pharmacy services are provided within a progressive medical group that has been utilizing clinical pharmacy specialists for over 20 years. Residents will gain experience in the delivery of team-based care in the medical home and other innovative models. Residents will manage a panel of physician-referred and proactively identified patients under robust clinical pharmacy agreements across a spectrum of primary care practice environments including clinic, telephonic, and ehealth. Along with direct patient care, residents will also develop leadership, practice management, and research skills in longitudinal experiences. This residency will provide the resident with the skills to be able to start or maintain their own ambulatory care practice. This is an ASHP Accredited PGY-2 residency; PGY1 completion is required.
