The PGY2 residency program in Transitions of Care (TOC) is designed to equip the resident with the skills necessary to function as a transitions of care pharmacist by managing all aspects of medication therapy in both inpatient and outpatient settings. The resident will gain experience in transitioning the patient from the inpatient to outpatient setting for a core set of disease states. In addition, the resident will gain understanding of different payers and assist patients with obtaining medications via prior authorization and patient assistance programs. The residency offers training opportunities in Internal Medicine, Ambulatory Care, Medication Therapy Management, TOC in the ED, Accountable Care Organization, and Pharmacy Administration. Teaching opportunities include didactic presentations and precepting pharmacy students.
